Lakeview Health Services, Inc. is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). We are required by law to protect the privacy of health information that may reveal your identity and to provide a copy of the Notice of Privacy Practices which describes our health information privacy practices.
Lakeview has policies in place to properly guard your protected health information. The Notice of Privacy Practices explains your rights and some of our responsibilities to help you. It also describes how we may use and disclose your medical information and how you can request an accounting of disclosures which identify certain other persons or organizations to whom we have disclosed your health information in accordance with applicable law and the protections afforded in this Notice of Privacy Practices.

The Health Insurance Portability and Accountability Act (HIPAA) of 1996 Privacy Rule requires a covered entity to provide notice of its privacy practices. The notice describes the ways in which the covered entity may use and disclose protected health information (PHI). It also provides the individuals’ rights, including filing a complaint if they believe their privacy rights have been violated.
