Lakeview was selected to pilot a new initiative funded through the Office of Mental Health for Enriched Crisis and Transitional Housing in Ontario County. This two year pilot program, which is the only program of this type in the region, provides hospital diversion and step down transitional housing for individuals coming out of inpatient hospital stays. This program consists of three beds located in Canandaigua and a team of staff which will include a blend of direct care, clinical management and peer staff.
According to its website, “OMH envisions the enriched crisis and transitional housing pilot initiative will help fill a critical gap in New York State’s service delivery system by providing recovery-based, person-centered alternatives to existing hospital-driven services. Services provided by the enriched crisis and transitional housing will include short-term respite care, clinical monitoring, connection or re-connection to other mental health services, and peer support”.
