Adult SPOA Services

Adult Single Point of Access (SPOA) facilitates access to services for adults aged 18 and older who have a diagnosis of serious mental illness and who are striving to remain healthy and stable in the community.

Lakeview Health Services coordinates the SPOA committee and the referral process
for the residents of Ontario and Seneca Counties. The SPOA committee will make referrals to suitable programs based on established eligibility criteria and identified need.  

Available Services Include:

  • Non-Medicaid Case Management *
  • Assertive Community Treatment (ACT)

 

Housing Options include:

  • Community Residences
  • Licensed Apartments
  • Independent Supportive Housing

To access services and housing, a clinician or other qualified service provider should complete and submit the county-specific SPOA Referral Form and attach the required clinical documentation. *

The Ontario-Seneca Referral Form is available:

*The list of required clinical documentation can be found on page two of the SPOA Referral Form
    under “Instructions and Checklist. Unfortunately, we cannot process the SPOA application
    unless all the required documentation is attached. 


Although Lakeview offers housing services in six counties, our adult SPOA process is for residents of Ontario and Seneca Counties only.  For information about the SPOA process outside of Ontario and Seneca Counties, please contact the county mental health clinic for the area in which you are seeking services.
