Non-Medicaid Case Management

Non-Medicaid Case Management services support both adults and children who would benefit from help coordinating and advocating for their care.

Driven by comprehensive assessments, the case manager will work together with the individual served to establish a person-centered service plan.  Meetings will occur at home or in the community at least two times per month working toward  based on the identified goals outlined within the service plan.

The services provided include but are not limited to:

  • Linkage to and accessing resources in the community
  • Support with housing
  • Socialization
  • Collaboration with providers
  • Assistance with DSS and Social Security benefits
  • Advocacy
  • Other services upon request

Adult Non-Medicaid Case Management services are available for individuals who reside in Ontario, Seneca and Tompkins Counties. Applicants must be age 18 and older and provide documented mental illness.

Children and Youth Non-Medicaid Case Management services are available for residents in Ontario and Seneca Counties. The services are for youth age 5-18 with a documented Severe Emotional Disturbance (SED).
