The Children and Youth Single Point of Access (SPOA) program is designed to help providers identify and assess children and youth who have significant mental health, behavioral, and related needs and to connect and coordinate their needs and treatment with existing community services.
Lakeview Health Services is responsible for implementation and management of the Single Point of Access (SPOA) program for community-based children and youth services for Ontario County.
The SPOA committee identifies children in crisis and who are at the highest risk of out-of-home placement and works to develop appropriate strategies to manage those children and keep them in their home communities, whenever possible. In coordination with schools and the other providers involved, the SPOA committee works to define a plan for success for the child/youth and family.
Services include care management, Functional Family Therapy Program (FFT), Multi-systemic Treatment Program (MST), Home and Community Based Services Waiver Program (HCBS), Home Based Crisis Intervention (HBCI), Child and Family Treatment Support Services (CFTSS), Community Residences (CR), Residential Treatment Facilities (RTF), Non-Medicaid Care Management and AspireHopeNY.
Based on established eligibility criteria and identified need, the SPOA will make referrals to the most suitable programs.
Services may be accessed by completing a SPOA referral form and supplying the required clinical documentation.
Referral forms may be completed and submitted by any individual but the form must be accompanied by the necessary clinical documentation showing a qualifying mental health diagnosis or considered to be emotionally disturbed.

Part 1 of the Referral Form must be completed by all applicants.
Part 2 of the Referral Form is only needed when referral to a Community Residence or Residential Treatment Facility is requested.
