West End Heights is a single site apartment program that has 20 one-bedroom apartments located in the West End Heights Apartment building.
This single site apartment program has 20 apartments all located in the West End Heights Building in Ithaca, NY. This is a transitional program designed to assist those with severe mental concerns. Individuals in the program receive medication oversight and one-on-one support with their recovery goals while working to acquire the skills to live as independently as possible. The program is designed for those who prefer to have staff available 24-hours a day, 7 days a week.
One-on-One Support
A primary Rehabilitation Coach (RC) will work individually with the resident at least once or twice a week to develop a person-centered service plan.
Medication Oversight
Assist resident with skills to store, order, and self-administer their medications.
Linkage to Community Supports
Assist the resident to link to resources in the community, which include care management, psychiatrist, mental health therapist, primary care physician (PCP), dentist, etc.
Community Outings
Staff provide opportunities for resident to participate in community activities such as going to sporting events, cultural festivals, recreational activities, etc.

As part of a Low-Income Housing Tax Credit (LIHTC) project, individuals are also subject to a financial/background screening process.
The West End Heights Apartment Program is licensed by the NYS Office of Mental Health.
