Lakeview Heights is a single site apartment program consisting of 26 one-bedroom apartments in Seneca Falls, NY
This is a transitional program designed to assist those with severe mental concerns. Individuals in the program receive medication oversight and one-on-one support with their recovery goals while working to acquire the skills to live as independently as possible. The program is designed for those who prefer to have staff available 24-hours a day, 7 days a week.
One-on-One Support
A primary Rehabilitation Coach (RC) will work individually with the resident at least once or twice a week to develop a person-centered service plan.
Medication Oversight
Assist resident with skills to store, order, and self-administer their medications.
Linkage to Community Supports
Assist the resident to link to resources in the community, which include care management, Psychiatrist, mental health therapist, primary care physician (PCP), dentist, etc.
Community Outings
Staff provide opportunities for resident to participate in community activities such as going to sporting events, cultural festivals, recreational activities, etc..

As part of a Low-Income Housing Tax Credit (LIHTC) project, individuals are also subject to a financial/background screening process.
Lakeview Heights is licensed by the New York State Office of Mental Health.
