Horizon Apartment Program

The Horizon Apartment Program supports your independence through weekly meetings while you are living in your own apartment.

Tompkins County

The Horizon Apartment Program has 21 transitional apartments scattered throughout Tompkins County with many located in the City of Ithaca, NY.  The program provides independent apartment living coupled with support for managing both mental and physical health and help with navigating community resources. The goal is to assist the resident to live as independently as possible in the community.

One-on-One Support  
A primary Rehabilitation Coach (RC) will work individually with the resident at least once or twice a week develop a person-centered service plan.

Medication Oversight  
Assist resident with skills to store, order, and self-administer their medications.

Linkage to Community Supports  
Assist the resident to link to resources in the community, which include Care management, Psychiatrist, mental health therapist, Primary Care Physician (PCP), dentist, etc.

Community Outings  
Staff provide opportunities for resident to participate in community activities such as going to sporting events, cultural festivals, recreational activities, etc.
