Community Oriented Recovery & Empowerment Services (CORE)
CORE Services can help individuals with living more independently, building meaningful social relationships, getting and keeping a job, going to school, or a training program and increasing overall wellness. Services are provided in-home or the community based on the individuals needs and preferences.
CORE Services are available for those enrolled in a Medicaid Managed Care Health and Recovery Plan (HARP) or Special Needs Plan (HIV–SNP) who are age 21 and older living with serious mental illness and/or substance use disorders.
Psychosocial Rehabilitation Services (PSR) services are designed to assist individuals to improve their independence to the greatest extent possible. Services are provided where the individual lives, works, learns, and socializes. These services may include rehabilitation counseling, skill building, and other interventions to support and develop skills to overcome barriers caused by mental health or substance use challenges. Individuals are supported and encouraged to be active and productive members of their families and communities. PSR also includes educational and employment supports to assist individuals in exploring, engaging in or maintaining their educational or employment goals.
Empowerment-Peer Supports are non-clinical, peer-delivered services with focus on rehabilitation, recovery, and resilience. The program promotes skills for coping with and managing behavioral health symptoms while facilitating the use of natural supports and community resources.
There is "No Wrong Door" to access CORE Services and no eligibility assessment is needed to qualify.
