Health Home Care Management isn’t a place… 

… it is a network of support for Medicaid recipients in the community

Care coordination to keep you healthy

Our Health Home Care Managers coordinate all the services a person needs to stay healthy and out of the hospital or emergency room.

 Working together with providers, care managers coordinate the necessary information to support your recovery goals. Services can include medical, behavioral and social care needs.

  • Help you identify your own goals and ensure everyone involved understands the care plan you created
  • Work with your providers to coordinate your care
  • Advocate on your behalf so you receive the care you deserve
  • Assist with navigating the complex healthcare system
  • Link you to resources for finding affordable housing, food, clothing, childcare and transportation
  • Link you to employment and legal support
  • Provide linkage to meaningful activities to improve your health

Who is Eligible?

To be eligible for Health Home services, a member must have one single qualifying condition or two chronic conditions:

One (1) single qualifying condition:

  • HIV/AIDS
  • Serious Mental Illness (SMI)
  • Sickle Cell Disease     
  • OR  Two (2) or more chronic conditions

One or more of the following risk factors must also be present:

  • Homelessness or risk of homelessness
  • Lack of social/family support
  • Non-adherence to treatments
  • Learning or cognitive issues
  • Deficits in activities of daily living
  • Recent release from incarceration or hospitalization
  • Probable risk for adverse events such as death, disability, inpatient, etc.
