The Ontario/Seneca Apartment program spans a two county area offering twelve scattered site apartments in Ontario County and eight scattered site apartments in Seneca County.
This program provides independent apartment living coupled with support services to assist individuals with managing their mental and physical health and navigating community resources.
The goal is to assist the resident to live as independently as possible in the community.
One-on-One Support
A primary Rehabilitation Coach (RC) will work individually with the resident at least once or twice a to week develop a person-centered service plan.
Medication Oversight
Assist resident with skills to store, order, and self-administer their medications.
Linkage to Community Supports
Assist the resident to link to resources in the community, which include Care management, Psychiatrist, mental health therapist, Primary Care Physician (PCP), dentist, etc.
Community Outings
Staff provide opportunities for resident to participate in community activities such as going to sporting events, cultural festivals, recreational activities, etc.
