Community Residences

Overview

Our Community Residence programs provide 24/7 support in a home-like setting for those struggling with mental health concerns. We offer assistance with all aspects of recovery with a goal to live as independently as possible in the community.

Your day-to-day living experience includes:

One-on-One Support      

Residents work individually with their personal Rehabilitation Coach (RC) to develop a person-centered service plan. They meet at least two to three times per week but more frequently, if needed.

Daily Living Skills

Staff and residents work together to acquire the skills and abilities needed to maintain the primary activities of daily life such as personal hygiene and grooming, preparation of food, cleaning and washing clothes, budgeting and money management.

Medication Oversight

Staff educate the residents on how to order, store and self-administer their medications. The goal is to help the resident to have a full carry order and manage their own medications. 

Linkage to Community Supports

Linkages to resources in the community such as care management, psychiatrist, mental health therapist, primary care physician, and dentist, are part of a regular wellness routine.

Community Outings

Opportunities for residents to participate in community activities such as attending sporting events, cultural festivals, recreational activities, and educational classes, etc. are ongoing.0

The Ontario Community Residence in Canandaigua, NY and Markham House Community Residence  in Ithaca, NY, are both 14-bed transitional housing programs with a home-like setting. Serving those with severe mental health concerns, the program provides assistance for all aspects of recovery with a goal to live as independently as possible in the community. Both community residences programs are licensed by the NYS Office of Mental Health.

The Single Resident Occupancy (SRO) in Ithaca, NY is designed to support those with severe mental health concerns and who have struggled living in other types of residential settings. The residents at the SRO receive medication oversight, one-on-one support with recovery goals, and staff that are available 24-hours a day, 7 days a week.  The SRO offers 38 studio apartments with comfortable living space and dining services for three meals per day.  The program is licensed by the NYS Office of Mental Health.
