Overview

Supportive Housing provides assistance to eligible individuals with securing and maintaining permanent independent affordable housing.  

We provide the services you need to get started on your journey to independent living!

Our Supportive Housing programs offer a variety of apartment opportunities for those who are likely to benefit from stable, permanent housing coupled with the advocacy to help you maintain healthy living.

Lakeview’s Supportive Housing programs are available to residents of:
Ontario, Seneca, Wayne, Yates, Livingston, and Tompkins Counties

  • Age 18 or over
  • Ability to live independently in the community
  • Adults experiencing intermittent homelessness
  • Substance Use Disorder
  • Persons living with HIV or AIDS
  • Health Home Enrollment or Referral 
  • High end users of Medicaid 
  • Income requirements
  • Meet HUD guidelines
